Bring your completed paperwork, a valid photo ID, and any relevant medical records to your first appointment.

Please Note: We are a private pay office and do not directly bill 3rd party insurance (except Traditional Medicare). We can provide an itemized bill for you to submit to your insurance upon request. Cancellation Policy: Please notify us at least 24 hours in advance if you need to reschedule to avoid a $50 fee.
